The Willis L. Miller Library is an official Passport Acceptance Facility. Please stop by the information desk to inquire about this service.

Passport applicants should bring

  • Proof of U.S. citizenship, such as an original or certified birth certificate or previous U.S. Passport. More specific information can be found here
  • A valid form of photo identification, such as a driver’s license.
  • One 2″x2″ passport photo (The Willis L. Miller Library does not take passport photographs.)
  • Passport fees, including check or money order for State Department fees and acceptance/execution fees.

The Library has passport application forms available, or you may download and print the forms from the U.S. Department of State.

Costs

Please note: Costs are determined by the U. S. Department of State. Only checks or money orders can be accepted as payment.

Adults (16 and older)

  • Passport Book $110 payable to U.S. Department of State (check or money order)
  • Passport Card $30 payable to U.S. Department of State (check or money order)

Children (15 years and under)

  • Passport Book $80 payable to U.S. Department of State (check or money order)
  • Passport Card $15 payable to U.S. Department of State (check or money order)

Passport Acceptance fee is $35 per application.

Optional Services

  • Expedite Fee – $60 per passport (check or money order)
  • Express Mail – $16.48 one way (check or money order)

Passport Photos

The Willis L. Miller Library does not take photographs.

PROCESSING TIME

According to the U.S. Department of State, the processing times for passports are:

Routine: 6 weeks

Expedited (extra charge): 3 weeks

Emergency Passport / Expedited at Agency: within 8 business days, based on need; restrictions apply

PLEASE COME PREPARED

  • Make an appointment with the library.
  • Completed, unsigned application form DS-11(PDF or complete online & print). The Library will have copies of the DS-11 available to complete too.
  • Proof of United States citizenship (original documents – no copies)
    • Original documents must be submitted, and will be returned to you by the U.S. Department of State. Acceptable evidence includes:
      U.S. Birth Certificate(must be original and list your full name, date & place of birth, and parent(s) full names), Naturalization Certificate, expired  U.S. Passport.
      Note: The U.S. Birth Certificate must be original and list your full name, date & place of birth, and parent(s) full names. See the Passport website for additional requirements.
    • If you need your birth certificate, contact the County Recorder or Registrar of Birth. You can also find this information using Vital Check at www.vitalchek.com.
  • Government issued I.D. as well as a photocopy of the front and back of the I.D.
  • One 2″ x 2″  photograph. (Library does not take photos, see list of locations near the library  that take passport photos)
  • Social Security Card
  • Two Checks or Money Orders for fees
  • For passport for minors, both parents and the child must appear and there are special additional requirements. Please see: Passports for Minors under 16 year old andPassports for Children Age 16 and 17.
